When you rightclick it you see two lights going from top symbol to bottom one. If you encounter Diablo when lights reach that bottom one he would be much stronger.

I hope it's the right explanation xd

Does the stronger version of diablo drop better items or is he just harder to kill?

Propably just more hp/armor/res/dmg... Drop stays the same. I think that only BrightLord could say more about it ;p
