Is that transformation random or do you control which type of creature you become ?

I just used keyboard 'W' click

Each transformation require a set or specific item

PS: 1080p is not available, only max 720

Mictlantecuhtli the demon blade for example. i think it is either executioner blade or colossus blade unique item.

Item has ilvl 77. Probably diablo hell on mp10 or late torment.
