use 1h+shield if you aren't barbarian, have a lot of HP so you don't get oneshot
capped resistances are important, high as possible armour class also helps

I think it's the same reason as for any mod based on vanilla Diablo - Hellfire was reverse engineered quite recently unlike original game. Though, Brightlord has written the whole code for the game by himself, making use of diabdat.mpq assets. That having said, I kinda don't know why he didn't build...

AFAIK they drop from high level items randomly with low chance. Like those you find on Torment difficulty (maybe ones on the end of Hell diff?)

Upon reaching lvl 20 I believe you are automatically able to enter 2nd difficulty. You can still fight diablo on previous one though if you want.

no it isn't a bug, light/leather type armour does not display on character models in diablo 1, only medium/mail and heavy/plate does display

You need archmage for reading books anyway, and you also can use it instead of spellweaver if you are using page 5 spells which do not benefit from spellweaver recipe.
