the qlvl are much high, I think hell levels of Torment is more probably, but requires lvl 50

This levels for entering are very high, I think 40-50 must be the maximum, 56 for terrors domain is very very high.

it will drop from
hell/terror domain 4 boses and lazarus
torment/hell 15,16 torment all unique mob.
i earned it from torment/domain unique knight.

yes. viz armor and belt drops well from hell/hell all unique mobs,
maybe viz retalition drops hell lazarus and two succubus/domain all unique mobs,offhand will drops from hell/diablo and torment/all unique under mobs 15 floor.
i earn it from tormnet,hell/diablo

edit:
even i mentioned that armor and belt drops well,its relative.
basically major items are extremely rare.
