Can anybody tell me how to play melee characters?

I've played all the characters so far and I seems having difficulties with playing all the melees. Every melee character got stuck at lvl 40 where I seems can't survive either in hell (dungeon 14+) or first lvl of torment. I tried mf with other chars and there's that lvl requirments while the gears I can use are not match for the bosses I face. How did u guys make it thru?

use 1h+shield if you aren't barbarian, have a lot of HP so you don't get oneshot
capped resistances are important, high as possible armour class also helps
pretty much that, yeah, throw like a third of your lvlup points into vitality and you'll see the difference

More points into DEX, less points into STR and some into VIT. Only put points into STR when you need to meet the requirements to wear or wield something better.

OK, new problem here...
I've dealt with warrior char now and can handle it pretty well. Now my question is: how do u guys deal with those monsters that kite u? With warrior I teleport to their face and take em down one by one. But char like barb... I really don't know how to now.
