This is a documented glitch. You should just be able to go through the quest again and it'll drop this time, but sometimes you need to do a different quest first to, uh, reset the quest log I guess?

How hard do you suppose it would be to add minion pump as an affix to items? "Skeleton damage +50%" and the like? That way you could simply choose how much of an active roll your minions are as you progress.

Right-click the Tchernobog icon you use to start the game, select 'run as administrator' and 'run in compatability mode' if you haven't already. Odds are what you're seeing is your system rejecting the game's request to start a few new files because it doesn't have the correct permissions yet.
