That is how it is supposed to work. The dungeons and quest encounters will respawn each new game with a fresh and different map. Completed quests will not repeat the rewards.

Tactics. Try to antagonize a few with a bow to draw them out a few at a time and switch weapons around a corner to ambush. Try taking out 3-5 at a time. When cave wall collapses and releases a hive of serpents, teleport away and again draw a few out at a time. Bottlenecks are your advantage here.

For all melee and archers, increased weapon speed is critically important for stun locking. After that, mana and life leach to keep skills active or in the case of Barbarian to keep you alive when taking elemental hits.
