Gems of better quality cannot be re-converted back into lesser quality gems, no. The 'transmute gem' recipes are relatively worthless; by the time you start finding recipes that require better materials you'll be finding equipment that gives you those materials anyway.

Welcome to the forums!
You can change your character's armor appearance like in Diablo 1 by equipping either light, medium, or heavy armor; if you're talking about going into the code and painting your own custom sprites that's too heady a subject to get into here.

I got both Cleglaw's sword and shield doing Lost Treasure runs in Normal difficulty with about 250 MF%. It took a long-ass time though.
