Heh. The mpq file can be from demo version of Diablo. Or some people here can live in non-english speaking countries. So they try to use spanish, german or russian-translated (and bugged) version

Yes. When you exit the game, then return, all levels re-generate. But all completed quests, activated teleports for each difficulty level saved, you can jump to 2,4,6,8 so on levels and continue playing. This is how belzebub works. If you waht to clear all levels in single run, you can press "P...

I Don't know how game calculates chances of getting uniques/whatever. But i found that belzebub engine have a variable containing number of steps made from start of the game. Some people write you loose some money when you die. Not sure about that. The gold stays on the place where you died. If infravision spell/murky pool effect was active when you die, effect remains active till you exit the game.
