I haven't seen an off-hand weapon yet and in fact have seen very few set items. I assume these come only at the higher difficulty settings.

I like the idea of an off-hand weapon class. That would let all PC classes be dual weilders!

Couldn't find any reference in the forum to importing characters from the same classes in original games. Is is possible? Maybe a sorceror but not a bard, for example.

Two possible bugs - First, I have never found the Butcher on level 1. Second, re Wirt's quest - the Butcher's Chamber - Wirt gives the quest backwards, but it lis essentially to cast the Town Portal spell when in the Chamber. I found what looks to be the Chamber on level 2 and cast the spell. Nothin...
